Parish Nurse Survey

8 Nov 2014 by SHRCChurch, Comments Off on Parish Nurse Survey

In every part of our country, parishioners, such as nurses, are devoting time and talent to help members of the parish community to improve health of body, mind and spirit.  In their ongoing effort to better serve the needs of the Sacred Heart Community, the Sacred Heart parish nurses have compiled a survey.  Click on this link Parish (Nurse) Survey (Rev2) 11-09-14 to download a printable version of the survey.  Your input is vital to determine the need for services in our Faith Community. Please take just a few minutes to complete this survey and return by November 23, 2014.

Thank you – Your Parish Nurses
